Terms & Limitations:

  • 1. Gold Star Dental MEMBERSHIP PLAN: This is a dental membership plan and is not to be considered a dental insurance plan. The dental membership plan is for patients without dental insurance and cannot be used in conjunction with a dental insurance plan.

    2. ADMITTANCE TO THE PLAN: Members may only be admitted to the plan upon initial signup with paid activation fee per member.

    3. CANCELLATION OF MEMBERSHIP: Notification of cancellation of the plan must be made 30 days prior to the renewal date. There are no refunds for the activation fee and subscription dues. Patients are not required to receive any recommended treatment that is diagnosed prior to using the benefits included with the membership. Payment of less than the plan’s duration of membership fees may result in being charged usual and customary fees for all services (including those already provided) and being charged the remaining months’ fees in a lump sum.

    4. NON-TRANSFERABLE: This plan is non-transferable and can only be used for registered members of the plan.

  • 3. USE OF PLAN AND SERVICES COVERED: This plan may be used at Gold Star Dental and applies only to dental procedures offered at 66o5 McCart Ave. Fort Worth, TX 76133. The membership plan includes services normally covered in the scope of general dentistry.

    2. DENTAL CARE NEEDS TOO COMPLEX: There may be dental care needs beyond the scope of services provided at Gold Star Dental office. Patients with specialty dental care needs will be advised as soon as possible and referred to the appropriate dental specialists. Referred services are not covered by this membership plan. After specialty services are performed, patients may return Gold Star Dental for treatment(s) covered under the Membership Plan.

  • SCHEDULING: Members can make appointment for their routine checkup by calling our office or directly online at www.goldstardentalpllc.com

    No-SHOW or LAST-MINUITE CANCELLATION: $25 to $50 fee (based on length of appointment) may be incurred for each broken appointment without a 24-hour advance notice.
